TINJAUAN FORMULIR PARTOGRAF BERDASARKAN ASPEK DESAIN FORMULIR DI RUMAH SAKIT ISLAM AMAL SEHAT SRAGEN

Abstract

ABSTRACT

 

Based on the survey still found 10 documents incompleteness of 7 document. Judging from the design of the form have represented any data needed by a midwife. So that the form must be filled with complete on all data items. Therefore, it requires an analysis of the design aspects of the form. The research objective was to determine the form and design aspects partograf.Jenis research is descriptive with cross sectional approach. The definition of this term is the physical aspect, the aspect of anatomy and content aspect. The object of this study partograf form. The research instrument is the observation guidelines. Analysis of descriptive data. The results showed partograf form of physical aspects are in accordance with the design aspects of the form seen of materials, shapes, sizes and colors. Aspects of anatomy seen from heading names and addresses of organizations is still too small. Body already contains a  title, organization name and logo but the font size is not big enough, while the cover / close requires a data item inspection date and place of service delivery data items. While the contents visible aspects of the item need additional item medical record number by replacing item no. RS / health center / RB, and sequent to address items not in accordance with the grouping that is in the form partograf. It can be concluded and advised that need additional instruction partograf filling in forms, increase the size of the letters on either title or name of the organization, adding items place and date of service delivery as well as address items placed in the group of patient identity.

ABSTRAK

 

Berdasarkan survei 10 dokumen masih dijumpai ketidaklengkapan sebanyak 7 dokumen. Dilihat dari desain for- mulirnya telah mewakili setiap data yang dibutuhkan oleh bidan. Sehingga formulir tersebut harus terisi dengan lengkap pada semua item data. Oleh karena itu memerlukan suatu analisis dari aspek desain formulir. Tujuan penelitian adalah mengetahui aspek desain formulir partograf. Jenis penelitian adalah deskriptif dengan pendeka- tan cross sectional. Definisi istilah ini adalah aspek fisik, aspek anatomi dan aspek isi. Objek penelitian ini for- mulir partograf. Instrumen penelitian adalah pedoman observasi. Analisis data secara deskriptif. Hasil penelitian menunjukkan formulir partograf dari aspek fisik sudah sesuai dengan aspek desain formulir dilihat dari bahan, bentuk, ukuran dan warna. Aspek anatomi dilihat dari heading nama dan alamat organisasi masih terlalukecil. Body sudahmemuat judul, nama organisasi dan logo namun ukuran hurufnya masih kurang besar, sedangkan ba- gian penutup/ close memerlukan item data tanggal selesai pemeriksaan dan item data tempat pemberian pelayanan. Sedangkan aspek isi dilihat dari item perlu penambahan item nomor rekam medis dengan menggantikan item no. RS/Puskesmas/RB, dan sequent untuk item alamat belum sesuai dengan grouping yang ada didalam formulir partograf. Maka dapat disimpulkan dan disarankan bahwa perlu penambahan instruksi pengisian dalam formulir partograf, menambah ukuran huruf pada judul maupun nama organisasi, penambahan item tempat dan tanggal pemberian pelayanan serta item alamat ditempatkan pada kelompok identitas pasien.
