AKURASI KODE DIAGNOSIS CHRONIC KIDNEY DISEASE BERDASARKAN ICD-10 PASIEN RAWAT INAP DI RSUD dr. SAYIDIMAN MAGETAN

Accuracy is the establishment of appropriate disease code, complete and in accordance with ICD-10. Based on the preliminary survey that researchers do, there are 6 document accurate patient medical records or 40% and 9 inaccurate documents or records as much as 60% of the 15 documents. This is due to an error coding combination with hyperten- sion diagnosis and coding of previous disease history. The purpose of this study to determine the accuracy of diagnosis codes Chronic Kidney Disease patients hospitalized in dr. Sayidiman Magetan. This type of research is descriptive and retrospective approach. The population in this study is a document medical records of hospitalized patients of Chronic Kidney Disease number 154 2015. The sampling technique systematic sampling, with a sample of 51 documents. The re- search instrument used observation and interview guides. Collecting data through observation and interviews. Data pro- cessing techniques, namely the collection, editing, classification, tabulation, narrative. Data analysis using descriptive. The results showed that the document is accurate as many as 21 documents (41,18%) and is not accurate as many as 30 documents (58,82%). Inaccuracies due to coding errors combined with a diagnosis of Hypertension, regular HD post code, using memory and sometimes open the ICD-10 volume 3. Conclusions from this research is the use of memory en- coding/rote, sometimes opening the ICD-10 volumes 3 and did not open the ICD-10 volumes of research 1. Suggestions are preferably reform Procedure and officers must consider the supporting information.

Akurasi adalah penetapan kode penyakit yang tepat, lengkap dan sesuai dengan ICD-10. Berdasarkan survey pendahu- luan yang peneliti lakukan terdapat 6 dokumen rekam medis pasien yang akurat atau 40% dan tidak akurat 9 dokumen rekam medis atau sebanyak 60% dari 15 dokumen. Hal ini disebabkan adanya kesalahan pengodean kombinasi dengan diagnosis Hypertensi dan pengodean dari riwayat penyakit terdahulu. Tujuan penelitian ini untuk mengetahui akurasi kode diagnosis Chronic Kidney Disease pasien rawat inap di RSUD dr. Sayidiman Magetan.Jenis penelitian ini adalah deskriptif dengan pendekatan retrospektif. Populasi dalam penelitian ini adalah dokumen rekam medis pasien rawat inap Chronic Kidney Disease sejumlah 154 tahun 2015. Teknik pengambilan sampel sampling sistematis, dengan sampel 51 dokumen. Instrumen penelitian menggunakan pedoman observasi dan pedoman wawancara. Cara pengumpulan data menggunakan observasi dan wawancara. Teknik pengolahan data yaitu pengumpulan, edit, klasifikasi, tabulasi, narasi. Analisis data menggunakan deskriptif. Hasil penelitian menunjukkan bahwa dokumen akurat sebanyak 21 dokumen (41,18 %) dan tidak akurat sebanyak 30 dokumen (58,82 %). Ketidakakuratan disebabkan karena kesalahan pengo- dean kombinasi dengan diagnosis Hypertensi, kode post HD rutin, menggunakan ingatan dan terkadang membuka ICD-10 volume 3. Simpulan dari penelitian ini adalah pengodean menggunakan ingatan/hafalan, terkadang membuka

 

ICD-10 volume 3 dan tidak membuka ICD-10 volume 1. Saran penelitian ini adalah sebaiknya melakukan pembaharuan

Prosedur Tetap dan petugas harus memperhatikan informasi penunjang.
