Abstract
Returns documents medical records of inpatient wards to medical records assembling parts there are not in
accordance with standard operating procedures. This is due to the completion of the medical record documents
exceeds a predetermined time limit diprosedur fixed at 2 x 24 hours. The purpose of this study is Knowing Returns
Procedure Document Flow Inpatient Medical Record First Quarter 2014 dr. Soehadi Sragen. This research is
descriptive, using qualitative approach. population is the officer responsible for the return of documents medical
records, totaling 12 officers, the procedure returns the document remains incomplete medical records, the procedure
remains the responsibility of filling the medical records of all members of the population and the sample made in
the sample. data collection by interview and observation. The instrument used is the observation and interview.
The data analysis is descriptive.
The results showed that the return of the document medical records of hospitalized patients there are not in accordance
with standard operating procedures. Completion time of documents in the medical record, the document is returned
late and exceeded the time limit specified in the standard procedure that is 2 x 24 hours. In addition, the use of the
book in the form of return incompleteness and medical record documents are not used anymore, so the completeness
of a document inpatient medical records can not be well documented. General Hospital dr. Soehadi Prijonegoro
Sragen should return the document in inpatient medical records conducted in accordance with standard operating
procedures that have been set by the hospital director. Book incompleteness and form should be used again, so
that the documents can be inpatient medical record is well documented. In addition, directors should review the
procedures for returning the document remains inpatient medical records, especially on the use of medical record
book less complete analysis.

dengan wawancara dan observasi. Instrumen yang digunakan adalah pedoman observasi dan wawancara. Analisis
data yang digunakan adalah deskriptif. Hasil penelitian menunjukkan bahwa dalam pengembalian dokumen
rekam medis pasien rawat inap masih ada yang belum sesuai dengan prosedur tetap. Dalam waktu pelengkapan
dokumen rekam medis, dokumen dikembalikan terlambat dan melebihi batas waktu yang ditentukan di prosedur
tetap yaitu 2 x 24 jam. Selain itu penggunaan buku ketidaklengkapan dan formulir dalam pengembalian dokumen rekam medis sudah tidak digunakan lagi, sehingga kelengkapan suatu dokumen rekam medis rawat inap tidak
dapat terdokumentasi dengan baik. Rumah Sakit Umum Daerah dr. Soehadi Prijonegoro Sragen sebaiknya dalam
pengembalian dokumen rekam medis rawat inap dilaksanakan sesuai dengan prosedur tetap yang telah di tetapkan
oleh direktur rumah sakit. Buku ketidaklengkapan dan formulir sebaiknya digunakan lagi, supaya kelengkapan
dokumen rekam medis rawat inap dapat terdokumentasi dengan baik. Selain itu direktur sebaiknya meninjau ulang
prosedur tetap tentang pengembalian dokumen rekam medis rawat inap khususnya pada penggunaan buku analisis
rekam medis kurang lengkap.
