ANALISIS KELENGKAPAN PENGISIAN FORMULIR RESUME MEDIS PADA KASUS ABORTUS IMMINENS

Based on the results of a preliminary survey document medical records of patients with a diagnosis of threatened abortion performed at the General Hospital Fitr Candra Wonogiri to 10 form Medical Resume found incompleteness in the review of the authentication based on the name of the doctor that there are six forms are not filled completely (60%) and 4 form Fully filled (40%). The purpose of this study is Knowing completeness of form Resume Medical Diagnosis threatened abortion in the General Hospital Fitr Candra Wonogiri This study was descriptive population of this study is to document medical records of hospitalized patients Diagnosis of threatened abortion in the General Hospital Fitr Candra Wonogiri 2015 ie 66 medical record document form. Sample determination technique using systematic random sample in this research as many as 66 medical record document form. Data collection using checklists and interview guides. Methods of data collection are observation and interview. Data processing techniques are collecting, editing, classifying, tabulating and presenting data. The type of data analysis is descriptive. The results showed that the highest percentage rate of completeness review document forms authentication is 66 Medical Record (84.84%) in the doctor’s signature item. The lowest percentage of completeness level of authentication review is 53 Medical Document Document form (80,30%) on doctor name item. Based on the research results suggested the need for guidance and cooperation between each section to complete the authentication charging the doctor’s name and signature of the doctor to complete in order to produce an accurate medical record documents

Berdasarkan hasil survey pendahuluan dokumen rekam medis pasien dengan Diagnosis Abortus Imminens yang dilakukan di Rumah Sakit Umum Fitri Candra Kabupaten Wonogiri terhadap 10 formulir Resume Medis ditemukan ketidaklengkapan dalam review autentifikasi berdasarkan nama dokter yaitu terdapat 6 formulir yang tidak terisi lengkap (60%) dan 4 formulir terisi dengan lengkap (40%). Tujuan penelitian ini adalah Mengetahui kelengkapan pengisian formulir Resume Medis Diagnosis Abortus Imminens di Rumah Sakit Umum Fitri Candra Kabupaten Wonogiri Jenis penelitian ini deskriptif Populasi dari penelitian ini adalah dokumen rekam medis pasien rawat inap Diagnosis Abortus Imminens di Rumah Sakit Umum Fitri Candra Kabupaten Wonogiri Tahun 2015 yaitu 66 formulir dokumen rekam medis. teknik penentuan sampel menggunakan sampel acak sistematis dalam penelitian ini sebanyak

66 formulir dokumen rekam medis. Pengumpulan data dengan menggunakan checklist dan pedoman wawancara. Cara pengumpulan data yaitu observasi dan wawancara. Teknik pengolahan data adalah collecting, editing, klasifikasi, tabulating dan penyajian data. Jenis analisis datanya yaitu deskriptif. Hasil penelitian menunjukan bahwa Persentase tertinggi tingkat kelengkapan pengisian review autentifikasi yaitu 66 formulir Dokumen Rekam Medis (84,84%) pada item tanda tangan dokter. Persentase terendah tingkat kelengkapan pengisian review autentifikasi sebesar 53 formulir Dokumen Rekam Medis (80,30%) pada item nama dokter. Berdasarkan hasil penelitian disarankan perlu adanya pengarahan dan kerja sama antar masing-masing bagian untuk melengkapi pengisian autentifikasi nama dokter dan tanda tangan dokter dengan lengkap agar menghasilkan dokumen rekam medis yang akurat.
