TINJAUAN KEAKURATAN KODE DIAGNOSIS UTAMA ABORTUS IMMINENS PADA DOKUMEN REKAM MEDIS PASIEN RAWAT INAP DI RUMAH SAKIT ISLAM KLATEN

Abstract

ABSTRACT

 

The accuracy of the code are affected by the determination of the patient’s diagnosis. If the diagnosis is inaccurate encodes it will affect the number of cases in the report preparation morbidity, mortality as well as the calculation of the various figures of Statistics Hospital. Based on the preliminary survey of 10 documents medical records of inpatients with a diagnosis of threatened abortion, there are four documents (40%) were inaccurate. The purpose of this study to determine the accuracy of diagnosis codes on the documents threatened abortion medical records of patients hospitalized in the Klaten Islamic Hospital.This type of research is descriptive, with a retrospective approach. The research instrument using unstructured interviews and sample observation method used is Sys- tematic Sampling at 83 medical records document the diagnosis of threatened abortion.The results showed that the primary diagnosis code on the document threatened abortion medical records of patients hospitalized in the Klaten Islamic Hospital accurate as many as 30 documents (73%), while that is not accurate as many as 11 docu- ments (27%). Preferably officier coder more careful in coding and writing diagnosis code, the selection of a block coder must specify the type of diagnosis statment before encoding in order to get the code right, and the clerk must thoroughly input the code of medical records to the computer because if something goes wrong input will affect the reporting of the index disease.

ABSTRAK

 

Keakuratan kode dipengaruhi oleh penentuan diagnosis pasien. Apabila dalam mengode diagnosis tidak akurat maka akan berpengaruh pada jumlah kasus dalam pembuatan laporan morbiditas, mortalitas serta penghitungan berbagai angka Statistik Rumah Sakit. Berdasarkan survey pendahuluan dari 10 dokumen rekam medis pasien rawat inap dengan diagnosis Abortus Imminens, terdapat 4 dokumen (40%) yang tidak akurat. Tujuan penelitian ini untuk mengetahui keakuratan kode diagnosis Abortus Imminens pada dokumen rekam medis pasien rawat inap di Rumah Sakit Islam Klaten. Jenis penelitian yang digunakan adalah deskriptif, dengan pendekatan retrospektif. Instrumen penelitian dengan menggunakan metode wawancara tidak terstruktur dan metode observasi Sampel yang digunakan yaitu Sistematis Sampling pada 83 dokumen rekam medis diagnosis Abortus Imminens. Hasil penelitian menunjukan bahwa kode diagnosis utama Abortus Imminens pada dokumen rekam medis pasien rawat inap di Rumah Sakit Islam Klaten yang akurat sebanyak 30 dokumen (73%), sedangkan yang tidak akurat seban- yak 11 dokumen (27%).Sebaiknya petugas coder lebih teliti dalam mengkode dan menuliskan kode diagnosis, pemilihan blok coder harus menentukan tipe pernyataan diagnosis dahulu sebelum mengkode agar mendapatkan kode yang tepat, dan petugas harus lebih teliti menginput kode dari rekam medis ke komputer karena jika terjadi salah input akan mempengaruhi pelaporan indeks penyakit.
