KELENGKAPAN INFORMASI PENUNJANG DALAM PENENTUAN KEAKURATAN KODE DIAGNOSIS FRACTURE HUMERUS PASIEN RAWAT INAP DI RSUD Dr. MOEWARDI

Supporting information is information that is very important to determine the accuracy of the code, the complete information that can be read by the coder, the more precise and accurate code generated. Based on the results of a preliminary survey of the 10 document medical records with a diagnosis of fracture of the humerus in dr. Moewardi years 2013-2015 showed that 30% complete and 70% incomplete at 40% there is no description of open / closed fracture and 30% there are no sheets anamnesa. The porpose this study was to determine the completeness of supporting information in determining the accuracy of diagnosis codes facture humerus. This type of research is descriptive retrospective study. Population is a document medical records of patients hospitalized with a diagnosis of fracture of the humerus in 2013-2015 with a sample size of 71 documents with the technique of sampling using saturated sample, the research instruments using checklists and interview guides, observation data collection and interviews, while for the data processing that is by way of collection (collecting), editing (editing), classification (classification), tabulation, data presentation and analysis of descriptive data. The results showed that completeness of the information supporting there are 54 DRM (85.72%) and incomplete documents as much as 17 DRM (23.94%), the highest completeness of the information on multiple fracture of 70 DRM (98.59%) and the lowest in the description of the type of fracture, namely

61 DRM (85.92%). As for accuracy, accurate code as much as 24 DRM (33.80%) and the code is not accurate 47 DRM (66.20%). Conclusions from this research is the completeness of the information supporting the highest supporting information regarding multiple fracture and low of supporting information regarding the type of fracture. Suggested medics fill out a form complete physical examination to help the coder to determine an accurate code.

Informasi penunjang adalah Informasi yang sangat penting untuk menentukan keakuratan kode, semakin lengkap informasi yang dapat dibaca oleh koder maka semakin tepat dan akurat sebuah kode yang dihasilkan. Berdasarkan hasil survey pendahuluan terhadap 10 dokumen rekam medis dengan diagnosis fracture humerus di RSUD dr. Moewardi tahun 2013-2015 menunjukan bahwa 30% lengkap dan 70% tidak lengkap yaitu 40% tidak terdapat keterangan open / closed fracture dan 30% tidak terdapat lembar anamnesa.Tujuan penelitian ini adalah mengetahui kelengkapan informasi penunjang dalam penentuan keakuratan kode diagnosis facture humerus. Jenis penelitian adalah deskriptif dengan pendekatan studi retrospektif. Populasi adalah dokumen rekam medis pasien rawat inap dengan diagnosis fracture humerus tahun 2013-2015 dengan besar sampel sebanyak 71 dokumen dengan teknik pengambilan sampel menggunakan teknik sample jenuh, instrumen penelitian menggunakan checklist dan pedoman wawancara, pengumpulan data secara observasi dan wawancara sedangkan untuk pengolahan data yaitu dengan cara pengumpulan (collecting), edit (editing), klasifikasi (classification), tabulasi, penyajian data dan Analisis data secara deskriptif. Hasil penelitian menunjukan bahwa dokumen lengkap terdapat 54 DRM (85,72%) dan  dokumen tidak lengkap 17 DRM (23,94%), kelengkapan

 

tertinggi pada keterangan multiple fracture yaitu 70 DRM (98,59%) dan terendah pada keterangan jenis fracture yaitu 61

DRM (85,92%). Sedangkan untuk keakuratan, kode akurat sebanyak 24 DRM (33,80%) dan kode tidak akurat 47 DRM (66,20%). Simpulan dari penelitian ini adalah kelengkapan informasi penunjang tertinggi yaitu informasi penunjang mengenai multiple fracture dan terendah yaitu informasi penunjang mengenai jenis fracture. Disarankan petugas medis mengisi formulir pemeriksaan fisik secara lengkap guna membantu koder dalam menentukan kode yang akurat.
