ANALISIS KUANTITATIF DOKUMEN REKAM MEDIS PASIEN RAWAT INAP KASUS CEREBRAL INFARCTION DI RSUD dr. SOEDIRAN MANGUN SUMARSO KABUPATEN WONOGIRI TAHUN 2014

Abstract

Abstract
Cerebral Infarction is one of the 10 diseases in RSUD dr. Soediran mangun Suroso Wonogiri. From 20 medical
record documents, there was 50% incompletion data identity filing and 45% doctor names and there was no fixed
line. The aim of this research is to know the document completion of inpatient medical record document in the case
of Cerebral Infarction. This research is descriptive research through retrospective approach. The research population
was inpatient medical record document with Cerebral Infarction case in RSUD dr. Soediran mangun Suroso
in Wonogiri regency in the fourth quarter which was 116 and the sample was 38 medical record document through
systematic sampling. The research instrument was checklist. The data were collected through observation and interview.
The data were analyzed descriptively. The result of the research shows that in the review form of entry and out,
anamneses, vital sigh graphic, clinic pathology laboratory, there is completion of identification covering patients’
name which is 38 9100%), complete medical number in the review of entry and out form which is 38 (100%), the
patients’ age in the anamnesis form and complete vital sign graphic which is 35 medical record documents (92.10%).
The authentication completion covers the signature of who is in charge in the complete form of entry and out review
which is 38 (100%). Correct documentation covers correcting mistakes which is 38 (100%), legible writing which is
38 (100%), and the correct fixed line which is 6 (15.78%). Based on the result of the research, there are still incomplete
medical record document and there is no fixed line in the free space. It is suggested that the SOP of medical
record completion analysis is completed with the fixed line in the free space so that the medical record complete
vand correct.

Abstrak
Cerebral Infarction merupakan salah satu dari 10 besar penyakit di RSUD dr. Soediran Mangun Sumarso Wonogiri
dan dari 20 dokumen rekam medis terdapat 50% ketidaklengkapan pengisian pada identitas pasien, 45% pada nama
dokter dan belum ada pemberian garis tetap. Tujuan penelitian mengetahui kelengkapan dokumen rekam medis pasien
rawat inap kasus Cerebral Infarction. Jenis penelitian ini deskriptif dengan pendekatan Retrospektif. Populasi
dalam penelitian ini adalah dokumen rekam medis pasien rawat inap dengan kasus cerebral infarction di RSUD dr.
Soediran Mangun Sumarso Kabupaten Wonogiri pada triwulan IV sebanyak 116 dan didapatkan sampel sebanyak 38
dokumen rekam medis dengan teknik pengambilan sampel secara Sistematis sampling. Instrumen dalam penelitian
dengan menggunakan checklist. Cara pengumpulan data dengan observasi dan wawancara.Dengan analisis secara
diskriptif. Hasil penelitian menunjukan bahwa pada formulir ringkasan masuk dan keluar, anamnesa, grafik vital
sign, laboratorium patologi klinik terdapat kelengkapan identifikasi meliputi nama pasien lengkap 38 (100%), nomor
rekam pada ringkasan masuk dan keluar lengkap 38 (100%), umur pasien pada formulir anamnesa dan grafik vital
sign lengkap 38 (100%), jenis kelamin pada formulir ringkasan masuk dan keluar lengkap 35 Dokumen Rekam Medis
(92,10%). Kelengkapan otentikasi meliputi tanda tangan penanggung jawab pada formulir ringkasan masuk dan
keluar, resume lengkap 38 (100%) dan nama terang pada formulir ringkasan masuk dan keluar, hasil pemeriksaan
laboratorium klinik lengkap 38 (100%). Kelengkapan pada pelaporan penting lengkap 38 (100%). Pendokumentasian
yang benar meliputi pembetulan kesalahan benar 38 (100%), penulisan jelas terbaca 38 (100%), pemberian
garis tetap benar 6 (15,78%). Berdasarkan hasil penelitian masih ada dokumen rekam medis yang belum lengkap dan pada dokumen rekam medis belum ada pemberian garis tetap pada area kosong. Disarankan perlu adanya pelatihan
analisis kuantitatif pada petugas rekam medis dan pembuatan prosedur tetap tentang pemberian garis tetap pada
area kosong.
