ANALISIS KUANTITATIF DOKUMEN REKAM MEDIS RAWAT INAP DENGAN DIAGNOSIS VERTIGO DI RSI AMAL SEHAT PERIODE TRIWULAN IV PADA TAHUN 2012

Abstract

ABSTRACT

RSI charity Health Sragen have procedures remain regarding the implementation of the charging document medical records. Disease Vertigo is a common disease and is a sequence of 7 of the top 10 diseases in the fourth quarter of 2012 is the month of October to December in the RSI charity Healthy Sragen. In charging documents medical records of patients with a diagnosis of vertigo on the reviews written are incomplete patient identiication (name of patient) obtained (70%) in sheet form for attachment results px. support, and referral sheet for attachment and care as much as 28 Document Medical Record. The purpose of the study was to determine the completeness of the medical record documents charging Inpatient Unit with a diagnosis Vertigo IV truwulan period in 2012 in the RSI Amal Sehat Klaten. This type of research uses descriptive research method. Methods of data collection using the check-List. The population is the entire document medical records of patients with a diagnosis of vertigo in the fourth quarter of 2012 in Healthy Sragen Charity RSI of 160 documents of medical records and obtained a sample of 40 medical records document the systematic sampling technique sampling. The results showed that completeness is the highest on the review patient identiication number (medical record), review critical reporting and review as many as 40 correct documentation Document Medical Record (100%) in the Gazette of the Entry and Exit Form. Based on the research results, it is suggested need to do a meeting or socializing and guidance to medical personnel, paramedics, and medical technicians on procedures remain No.30.05.S1.P1 on charging Responsibilities Inpatient Medical Records to document medical records illed with precise, clear, correct it with authority and responsibility.

ABSTRAK

RSI Amal Sehat Sragen telah mempunyai prosedur tetap mengenai pelaksanaan pengisian dokumen rekam medis. Penyakit Vertigo merupakan penyakit yang sering dijumpai dan merupakan urutan ke 7 dari 10 besar penyakit pada periode triwulan Iv tahun 2012 yaitu pada bulan Oktober – Desember di RSI Amal Sehat Sragen. Dalam pengisian dokumen rekam medis pasien dengan diagnosis Vertigo ditulis kurang lengkap pada review identiikasi pasien (nama pasien) didapat (70%) pada Formulir Lembar untuk penempelan hasil px. penunjang, dan lembar untuk penempelan surat rujukan dan perawatan sebanyak 28 Dokumen Rekam Medis. Tujuan penelitian adalah mengetahui kelengkapan pengisian dokumen rekam medis Unit Rawat Inap dengan diagnosis Vertigo periode truwulan Iv pada tahun 2012 di RSI Amal Sehat Sragen. Jenis penelitian menggunakan metode penelitian deskriptif. Metode pengumpulan data menggunakan check-List. Populasi adalah seluruh dokumen rekam medis pasien dengan diagnosis Vertigo periode triwulan Iv pada tahun 2012 di RSI Amal Sehat Sragen sebesar 160 dokumen rekam medis dan didapat sampel sebanyak 40 dokumen rekam medis dengan teknik pengambilan sampel secara sistematis sampling. Hasil penelitian menunjukkan bahwa kelengkapan tertinggi adalah pada review identiikasi pasien (nomor rekam medis), review pelaporan penting dan review pendokumentasian yang benar sebanyak 40 Dokumen Rekam Medis (100%) pada Formulir Lembaran Masuk dan Keluar. Berdasarkan hasil penelitian maka disarankan perlu dilakukannya rapat atau sosialisasi dan pengarahan untuk tenaga medis, paramedis, serta teknisi medis mengenai prosedur tetap No.30.05.S1.P1 tentang Tanggung Jawab Pengisian Rekam Medis Rawat Inap agar dokumen rekam medis terisi tepat, jelas, benar sesuai dengan kewenangan dan tanggung jawabnya.
